UC Davis Health System: A Comprehensive Approach to COPD Care Coordination

Overview

 

UC Davis Health includes a nationally renowned academic medical center and offers primary and specialty care outpatient services with offices located throughout the Sacramento region and on the UC Davis Sacramento campus.1-5

The Challenge

 

UC Davis Health recognized that COPD would be best managed with a multidisciplinary care team and wanted to develop a coordinated care approach that addresses COPD management across the inpatient and outpatient settings.6

The Approach

 

ROAD program: respiratory therapy team in the inpatient setting

  • Provided assessment, education, and coordination of discharge care for patients with COPD7

      Enrolled patients upon hospital admission6

      Developed a preferred treatment plan based on the GOLD grade algorithm for each patient8

      Provided one-on-one patient education in the inpatient setting and continued to support patients through postdischarge6,8

Comprehensive COPD Clinic: multidisciplinary team in the outpatient setting7

  • Managed patients based on their needs and disease type with optimized medications and delivery service8
  • Created a COPD-specific checklist as a semi-structured note within the EHR to ensure all providers collect the same information on each patient8

Outcomes

 

The ROAD program was associated with reduced readmission rates and length of stay6

  • The COPD-related readmission rate fell from 16% in 2011 to 6.8% in 2018
  • Shortened the average hospital length of stay for patients with COPD from 7.6 days to 5.4 days
