Mount Sinai: Enhancing Its Hospital-at-Home Program to Clear Beds for COVID-Positive Patients and Expand Treatment for Chronic Illnesses

Overview

 

Mount Sinai, an integrated delivery network in New York City, identified and appropriately treated patients in greatest need of inpatient COVID-19 and non-COVID-19 treatment and services.1

The Challenge

 

Mount Sinai needed to manage and accommodate the surge of COVID-19-positive cases while also maintaining care for other high-risk or rising-risk patients.2

The Approach

 

Partnerships and service-focused care enabled vulnerable patients to receive home-based care 1

  • Home care agencies ensured capacity for general monitoring, IV antibiotic administration, lab testing, physician monitoring, etc
  • Payor contracts were negotiated to provide maximum coverage

Outcomes

 
  • Doubled number of free beds pre-pandemic2
  • Negotiated contracts with nearly all payors to increase coverage for home care services 1
